Participation involves physical activity and inherent risk. By participating, our team voluntarily assumes all risks associated with the event and released Fxar Out Ministries, its board, volunteers, the venue and event organizers from liability for injury, loss or damages arising from participation.

This is a drug, alcohol, vape and smoke free event. Any participant acting inappropriately or violating these rules may be removed from the event without refund.

I am responsible for ensuring my team is present and ready for scheduled games. Games will run on schedule, no leniency will be given for late teams and teams may play with fewer players if necessary.

I will be the primary contact for the team, and all event communication, updates, schedules and important information will be sent directly to the email on this form. It is my responsibility to communicate this information to my team.
